In what situations do we process your data?

Your data is processed when you complete the contact form on our website and, in this way, send us an inquiry regarding the services offered, wish to schedule an appointment, or contact us for another purpose related to our operations.

What data do we process?

We process only the personal data you choose to provide. This may include your first and last name, email address, phone number, and any additional information included in the message you submit via the form.

Please do not include detailed health information or other sensitive data in your message unless it is necessary to handle your inquiry. If you provide such information, we will treat it with particular care.

What is our legal basis for processing your data?

By processing the data you provide in the contact form, we rely on Art. 6(1)(f) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation – GDPR), i.e., our legitimate interest in enabling contact with individuals interested in our services, responding to inquiries, and conducting correspondence.

How long do we store your data?

Contact details and the content of correspondence are stored for the period necessary to handle the inquiry and for a maximum of 12 months thereafter – to ensure continuity of communication and the ability to demonstrate its course.

If you enter into cooperation with us (e.g., schedule an appointment), the data may continue to be processed as part of the patient documentation – in accordance with separate regulations.

Who may receive your data?

Authorized employees and collaborators of NURT Centrum Terapii, as well as IT and hosting service providers supporting the maintenance of the website – acting under data processing agreements.

We do not share your data with other entities for marketing purposes.

Do we transfer data outside the European Economic Area (EEA)?

As a rule – no. If we use IT tools (e.g., email systems) that may involve transferring data outside the EEA, we ensure the safeguards required by the GDPR.

What are your rights?

Remember that at any time you can:

Do we make automated decisions or profile you?

No. Within the contact form, we do not perform profiling or automated decision-making.
